APPOINTMENTS
To schedule an appointment:
Call (972) 772-4644. Press ‘2’ at the prompt.
Please bring to every appointment (important):
1) All of your medications or an up-to-date medication list*
2) Insurance card
3) Photo ID
4) Co-payment
Appointment CancellationS
The appointment time that you schedule is yours and yours alone. We do not double or triple book appointments.
Should you need to cancel or change the date of your appointment, we require at least 24-hours notice as a courtesy to other patients seeking medical treatment. A dropped appointment is lost to everyone.
If less than 24-hours notice is given or if you miss an appointment, then a late cancellation fee or missed appointment fee will apply:
LATE CANCELLATION FEE: $25
MISSED APPOINTMENT FEE: $50
Please be aware that Monday appointments must be cancelled on or before Friday at noon. They cannot be cancelled by voicemail on a Saturday or Sunday while the office is closed.
Insurance companies will not pay for missed appointments; therefore, this fee must be resolved before the next appointment can be scheduled.
Late Arrivals Policy
If you arrive more than 15 minutes past your scheduled appointment time, you will still be seen. In fairness to other patients, however, you will likely need to wait until there is a gap between scheduled patients or after the scheduled patients have been seen. The front desk personnel cannot accurately predict how long the wait will be. Really. They simply can’t.
Please understand that if you are late and choose to cancel your appointment, a late cancellation fee will apply.
Notes
MEDICATIONS or MEDICATION LIST
Please bring ALL of your medications with you to each visit.
• Medications should be in their original, pharmacy-dispensed containers
• Bring your prescription and non-prescription medications, as well as any vitamins or supplements you are taking.
• Certain substances may not work as well or may cause problems when taken in combination with others. It is important for us to know everything that you are taking.
You may instead bring an up-to-date, legible medication list. Our med list form is available here. The list must include the following information:
1) DRUG NAME (Brand name and generic, if known)
2) DOSAGE [Strength] (mg, mcg, units, mg/ml, etc)
3) FORM (tablet, liquid, puffer, drops, suppository, injection, etc)
4) ROUTE [How the drug is taken] (mouth, ear, eye, rectal, etc)
5) SCHEDULE [When taken] (once daily, before meals, bedtime)
6) INDICATION [Why the drug is taken] (example: blood pressure)
7) PRESCRIBER [Who prescribed the drug]
If you forget to bring your medications or a complete, up-to-date medication list with you for your appointment, you may either:
a) reschedule your appointment for another day (please be aware that a late cancellation fee will apply); or
b) complete a new medication list before seeing the doctor, if you know all of the above information for the medications you take.
INSURANCE CARD
Bring your insurance card with your to every visit – we need this information to verify your insurance and to file a claim for your visit.
We verify insurance coverage every 30 days for all patients.
Please be aware that if we are unable to verify your insurance coverage, you must provide a deposit in advance of services.
If your insurance changes, please let us know at least 48 hours prior to your next appointment. Contacting an insurance company and verifying patient benefits can take anywhere from 10 to 45 minutes.
PHOTO IDENTIFICATION
Any state or federally-issued photo identification will do.
CO-PAY
All co-pays and deductibles must be paid at the time of your visit; this is mandated by Medicare and private insurance companies as part of their contracts.
In addition to cash or personal checks, we also accept MasterCard and VISA.
