Phone Consults 

While in general we prefer to evaluate patients and discuss treatment options in our office, occasionally there are incidences where we will feel it is appropriate to give advice and treat simple problems over the phone.

Phone consultations are offered as a convenience to patients who find it difficult to personally attend an appointment at the clinic.  They are typically 10 to 40 minutes in length with an additional 5 to 10 minutes of post-call documentation.  They are available to patients who have had an office appointment within the last year.

The standard rate for consult services is a $20.00 flat fee plus $2.50 per minute.

For example:  15 min = $57.50    30 min = $95.00

Phone consults are not billable to insurance companies.

Scheduling a Phone Consultation

1) Complete a PHONE CONSULTATION CONSENT FORM and fax or mail it to us.

2) Complete a PHONE CONSULTATION INTERVIEW FORM and fax or mail it to us.

3) Call our office to schedule your consult time.

      • a deposit of $50 will be charged to hold your appointment.

4) The physician will call you at the time of your consult.
